I'm surprised its taken you this long to start wondering that, considering the context. Lol.
Much like with humans, it would be measured via waist circumference and length from butt to mid back.
Could a diaper handle a horse? Realistically, absolutely. Although it'd have to have one hell of a poop capacity raiting, and some extreme absorbency, unless you want to be changing them every 4 hours. A large leak guard in the front would work well for stallions to stop their penis from poking out when they pee, (Stallions get erections when they pee.) And for mares, extra absorbency under the tail, due to the positioning of their vulva, would be needed to absorb as much as possible before it starts leaking into the front.
As far as poop, at least 1 foot tall leak guards, reinforced would be required. Horses poop a LOT, and almost every hour. Depending on the day, its mostly solid to squishy muck if healthy, but if the horse has diarrhea, you definitely will need the extra stopping power.
Only way you could clean em is with a hose if they've used their diaper, which could become a hassle since they're so big.
One last thing a proper, adorable looking horse diaper would need is reinforced, hook and loop tapes/or velcro tapes, and strong plastic or cloth covering. Horses are still animals, and enjoy playing around, rolling in the dirt, etc. They'd need to be pretty heavy duty, which luckily, mine are~
Overall, they'd need to be easy and comfortable to use and wear, cause minimal discomfort or distraction, and be near tear resistant. Recommend 200,000mL absorbancy rating for stallions and geldings, then an 195,000mL rating for mares. More absorbance for long sessions should be a priority, because as well as pooping their body weight, they pee a LOT too.
Definitely need to hold at least 10lbs of poop, and when used, they'd likely weigh close to 100 pounds depending on usage.
But yeah, I've got a few "blueprints" for horse diapers, one I myself use, and it is.. mostly effective~ but with how much I poop, its hard to find a diaper that'll contain it for long.

This diaper design, does have an opening, in the correct location, for the horse's tail, to be outside of the diaper (tails have a few distinct functions, they need to preform)??
If I were a shape shifter I'd definitely have fun hanging out with Spirit in diapers and hobbles.
He looks comfortable yet confused he's getting to test out furistostic diapers, and how U.S. military is over a century ahead in some areas of technology. lol
